ARM: kirkwood: remove KW_DEFADR_PCI_IO_REMAP

KW_DEFADR_PCI_IO_REMAP has the same value as KW_DEFADR_PCI_IO and is
only used to set up a 1:1 mapping. Remove it and update the mapping to
use KW_DEFADR_PCI_IO.
